§ 37-4-301

37-4-301. Qualifications -- fees. Applicants for licensure shall submit an application that must include, when required: (1) certification of successful completion of the national board examination; (2) certification of successful completion of a regional board examination; (3) three affidavits of good moral character; (4) a certificate of graduation from a board-approved dental school; and (5) an application fee.
