42-5-104. Content of notice. The notice required by 42-5-103 must contain: (1) the caption of the petition; (2) the address and telephone number of the court in which the petition is pending; (3) a concise summary of the relief requested in the petition; (4) the name, mailing address, and telephone number of the petitioner or the petitioner's attorney; and (5) a statement of the method of responding to the notice and the consequences of failure to respond.
