50-50-203. Application for license or permit. (1) Except as provided in subsection (2), an application for a retail food establishment license must be: (a) made to the department on forms and contain information required by the department; or (b) filed using an application for a license that is in compliance with rules established by the board of review established in 30-16-302. (2) An application for a temporary food establishment permit must be made to the local regulatory authority on a state-approved form. If a local board of review exists, the local board of review shall work with the state to provide a permit application under this subsection similar to the state-approved form, and the temporary food establishment may use the local board of review permit application.
