The summons shall be accompanied by an affidavit of the plaintiff or the plaintiff’s agent, representative or attorney, that the judgment, or some part thereof, remains unsatisfied, and shall specify the amount due thereon. [1911 CPA § 303; RL § 5245; NCL § 8801]
