'Provider' means a: 1. Person who has applied to participate or who participates in the Plan as the provider of goods or services; or 2. Private insurance carrier, health care cooperative or alliance, health maintenance organization, insurer, organization, entity, association, affiliation or person, who contracts to provide or provides goods or services that are reimbursed by or are a required benefit of the Plan.
