The governing body shall submit annually to the licensing authority or its designee: 1. The name, address, contact information, position held on the governing body and any other information required by the licensing authority of each member of the governing body; 2. A copy of the articles of incorporation, constitution and bylaws of the foster care agency; 3. Evidence satisfactory to the licensing authority that the foster care agency has the ability to financially support and sustain its activities, which may include, without limitation, financial statements and budgets; 4. A report from an independent auditor of the complete financial information for the foster care agency for the immediately preceding fiscal year; 5. A statement of purpose; and 6. An organizational chart or other chart that sets forth the structure of the foster care agency which includes, without limitation, a job description for each position listed in the chart.
