When an application is made by an indigent person to the board of county commissioners of any county for relief, the board of county commissioners shall require the indigent person to: 1. Provide the information necessary to determine the indigent person’s county of residence or nonresident status. 2. Provide the information necessary to determine the indigent person’s financial condition and eligibility for medical and financial assistance. 3.
