1. When any certificate of birth of a living child is presented without the statement of the given name, the local health officer, the local registrar or the State Registrar shall make out and deliver to the parents of the child a special blank for the supplemental report of the given name of the child, which shall be filled out as directed and returned to the State Registrar as soon as the child shall have been named. 2. The Board shall prescribe by regulation the time within which a supplementary report furnishing information omitted on the original certificate may be returned for the purpose of completing the original certificate. 3. Certificates of birth completed by a supplementary report shall not be considered as 'delayed' or 'altered.' [15:199:1911; A 1941, 381; 1931 NCL § 5249]
