Statute 449a 445

1. A declarant may revoke a declaration at any time and in any manner, without regard to his or her mental or physical condition. A revocation is effective upon its communication to the attending physician or other provider of health care by the declarant or a witness to the revocation. 2. The attending physician or other provider of health care shall make the revocation a part of the declarant’s medical record. (Added to NRS by 1977, 760; A 1987, 1309; 1991, 635)
