'Qualified patient' means: 1. A patient 18 years of age or older who has been determined by the patient’s attending physician or attending advanced practice registered nurse to be in a terminal condition and who: (a) Has executed a declaration in accordance with the requirements of NRS 449A.433; (b) Has executed a Provider Order for Life-Sustaining Treatment form pursuant to NRS 449A.500 to 449A.581, inclusive, if the form provides that the patient is not to receive life-resuscitating treatment; or (c) Has been issued a do-not-resuscitate order pursuant to NRS 450B.510. 2. A patient who is less than 18 years of age and who: (a) Has been determined by the patient’s attending physician or attending advanced practice registered nurse to be in a terminal condition; and (b) Has executed a Provider Order for Life-Sustaining Treatment form pursuant to NRS 449A.500 to 449A.581, inclusive, if the form provides that the patient is not to receive life-resuscitating treatment or has been issued a do-not-resuscitate order pursuant to NRS 450B.510.
