1. The poison book shall be in form substantially as follows: Name of Kind and Purpose Signature of Signature of Date Purchaser Residence Quantity of Use Pharmacist Purchaser 2. This book shall always be open for inspection by authorized officers of the law acting in their official capacity, and shall be preserved for at least 5 years after the date of the last entry therein.
