The Department shall, upon request of the Division of Welfare and Supportive Services of the Department of Health and Human Services, submit to the Division of Welfare and Supportive Services the name, address and social security number of each person who holds a permit or license issued pursuant to NRS 503.582 or 503.583 and any pertinent changes in that information.
