The application for the permit required by NRS 584.180 must: 1. Be submitted in writing on a form prescribed by the Director. 2. Include the name and address of the applicant, and, if a partnership, the names and addresses of all partners, and, if a corporation, association or other organization, the names and addresses of the president, vice president, secretary and managing officers. 3. Specify the name and location of the premises or business for which the license is sought. 4. Contain such other information as may be reasonably required by the Director.
