1. The Labor Commissioner shall prepare forms and blanks for the purpose of gathering the information and statistics required by this chapter, and may require any person to give the information and statistical detail designated in those forms. 2. Any person who refuses to furnish such detail and statistics in the form required by the Labor Commissioner is guilty of a misdemeanor and shall be punished by a fine of not more than $500. 3. In addition to any other remedy or penalty, the Labor Commissioner may impose against the person an administrative penalty of not more than $500 for each such violation.
