On or before September 1 of each year: 1. An insurer shall distribute to each employer that it insures any form for reporting injuries that has been revised within the previous 12 months. 2. The Administrator shall make available to physicians, chiropractic physicians, physician assistants and advanced practice registered nurses any form for reporting injuries that has been revised within the previous 12 months.
