The following acts, among others, are grounds for disciplinary action: 1. Failure of a licensee to maintain his or her facilities and premises in a clean and sanitary condition; 2. Failure to maintain records relating to the diagnosis, treatment and care of an animal; 3. Altering the records of an animal; 4. Making or filing a report which the licensee knows to be false; 5. Willful failure to report any dangerous, infectious or contagious disease or the results of any medical test as required by law; or 6. Willfully committing any inhumane or cruel act on any animal.
