'Policy of provider stop-loss insurance' means a policy of stop-loss insurance which: 1. Is issued to a provider of health care or a network; 2. Provides coverage for losses of the provider of health care or network above an attachment point which is stated in the policy; and 3. Covers losses of the provider of health care or network which result from the financial risk assumed by the provider of health care or network in a managed care contract with another insurer, including, without limitation, an accident and health insurer, health insurer, health maintenance organization or self-funded group health plan, with whom the provider of health care or network has entered into a contract to provide health care services.
