1. For the purposes of determining the period of creditable coverage of a person accumulated under a health benefit plan or group health insurance, the insurer shall provide written certification of coverage on a form prescribed by the Commissioner to the person which certifies the length of: (a) The period of creditable coverage that the person accumulated under the plan and any coverage under any provision of the Consolidated Omnibus Budget Reconciliation Act of 1985, as that act existed on July 16, 1997, relating to the continuation of coverage; and (b) Any waiting and affiliation period imposed on the person pursuant to that coverage. 2. The certification of coverage must be provided to the person who was insured: (a) At the time that the person ceases to be covered under the plan, if the person does not otherwise become covered under any provision of the Consolidated Omnibus Budget Reconciliation Act of 1985, as that act existed on July 16, 1997, relating to the continuation of coverage; (b) If the person becomes covered under such a provision, at the time that the person ceases to be covered by that provision; and (c) Upon request, if the request is made not later than 24
