If more than one class of risk is included: 1. A hospital or medical or dental service contract or evidence of coverage under a group or nongroup contract must not be issued or delivered in this state until a copy of the form of the contract is filed with the Commissioner and either: (a) Thirty days expires without notice from the Commissioner after the copy is filed; or (b) The Commissioner gives written approval before that time. 2. A schedule of premium rates to be paid by subscribers under either a group or nongroup contract must not be issued, delivered or used by any nonprofit hospital, medical or dental service corporation until that corporation files with the Commissioner a copy of the schedule together with any supplementary information required by the Commissioner and either: (a) Thirty days expires without notice from the Commissioner after the copy is filed; or (b) The Commissioner gives written approval before that time.
