An application for a permit for a new operation as a contract motor carrier must: 1. Be made to the Authority in writing. 2. Be in such form and be accompanied by such information as the Authority may require. 3. Include a complete set of the fingerprints of each natural person who is identified by the Authority as a significant principal, partner, officer, manager, member, director or trustee of the applicant and written permission authorizing the Authority to forward the fingerprints to the Central Repository for Nevada Records of Criminal History for submission to the Federal Bureau of Investigation for its report.
