5-C:57 Application for and Preparation of a Certified Copy of a Marriage Certificate. – I. The following information shall be provided on an application for certified copy of marriage record, before a certified copy is issued: (a) The groom's full name and the bride's full maiden name. (b) The date of marriage. (c) The place of marriage. (d) The purpose for which the certificate is requested. (e) The applicant's signature. (f) The applicant's relationship to the bride and groom. (g) The applicant's name, address, and telephone number. II. The applicant shall demonstrate a direct and tangible interest in the record. Source. 2005, 268:1, eff. Jan. 1, 2006.
