263:90 Application for Commercial Driver License. – I. The application for a commercial driver license or commercial driver learner permit shall include the following: (a) The full name and current mailing and residential address of the license applicant; (b) A physical description of the applicant, including gender indicated by checking either 'M' for 'male,' 'F' for 'female,' or 'X' for 'other,' height, weight, eye and hair color; (c) Date of birth; (d) Applicant's social security number, unless the application is made by the resident of a foreign jurisdiction for a commercial driver license; (e) Applicant's signature; (f) Certifications of driver qualification; (g) Any other information required by the department. II. The application shall be accompanied by the fee provided in RSA 263:42. III. When a licensee changes his name, mailing address, or residence, he shall notify the department as provided in RSA 263:9. IV. No person who has been a resident of the state of New Hampshire for 30 days may drive a commercial motor vehicle under the authority of a commercial driver license issued by another jurisdiction.
