281-A:20 Contents of Notice. – Notice of injury in writing on a form prescribed by the commissioner shall apprise the employer of the injury and shall state the name and address of the worker injured and the date and place of the accident. The employee shall execute the notice in duplicate and sign both copies. The employer shall acknowledge receipt of the notice by signing on both the original and the duplicate, and the employee shall retain the duplicate. Source. 1988, 194:2, eff. July 1, 1989.
