26:2H-5.1d Identification numbers of physicians publicly available. 2. The commissioner shall make publicly available the identification number for the physician or physicians, as applicable, that appear on hospital billing forms and billing forms of ambulatory care facilities licensed to provide surgical services, to the extent that doing so is consistent with the 'Health Insurance Portability and Accountability Act of 1996,' Pub.L.104-191.
