A. All dental licenses issued by the board shall bear:(1) a serial number;(2) the full name of the licensee;(3) the date of issue;(4) the seal of the board;(5) if the license is a specialty license, the specialty to which practice is limited;(6) the signatures of a majority of the board members; and(7) the attestation of the board president and secretary.B. All dental therapy licenses issued by the board shall bear:(1) a serial number;(2) the full name of the licensee;(3) the date of issue;(4) the seal of the board;(5) the signatures of a majority of the board members; and(6) the attestation of the board president and secretary.C. All dental hygienist licenses issued by the board shall bear:(1) a serial number;(2) the full name of the licensee;(3) the date of issue;(4) the seal of the board;(5) the signatures of a majority of the committee members; and(6) the attestation of the board president and secretary.D.
