Application for an escort certificate shall be made in the manner and form prescribed by the commissioner. The fee prescribed by law shall be submitted with such application. The applicant shall be a minimum of twenty-one years of age, possess a valid driver's license, furnish such proof of fitness and identity as required by the commissioner and submit to and pass a test administered by the commissioner.
