\nEvery group through its board, TPA, service company, agents, or other representatives shall require, before accepting an application, each applicant for membership to acknowledge in writing that the applicant has received the following:\n(1) A document disclosing that the members are jointly and severally liable for the obligations of the group.\n(2) A copy of the group's plan of assessment.\n(3) The amount of specific and aggregate stop loss or excess insurance or reinsurance carried by the group, the amount and kind of risk retained by the group, and the name and rating of the insurer providing stop loss, excess insurance, or reinsurance.
