\nEvery person fitting and selling a hearing aid, be it new or used, in the State of North Carolina, at or before the time of delivery of the hearing aid shall render to the user and/or purchaser a statement of sale to include the following:\n(1) Date of delivery\n(2) Condition of hearing aid; new, used, reconditioned\n(3) Hearing aid identification number\n(4) Name of manufacturer\n(5) Price of hearing aid\n(6) Charge for fitting and service\n(7) Name of dealer and/or fitter\n(8) Signature of customer.
