Effective: September 29, 2017 Latest Legislation: House Bill 49 - 132nd General Assembly A pharmacist who fails to make application to the state board of pharmacy for a license renewal within a period of three years from the expiration of the license must pass an examination for licensure and comply with sections 4776.01 to 4776.04 of the Revised Code; except that a pharmacist whose license has expired, but who has continually practiced pharmacy in another state under a license issued by the authority of that state, may obtain a renewed license upon payment to the executive director of the board the fee designated under division (A)(6) of section 4729.15 of the Revised Code.
