Effective: October 17, 2019 Latest Legislation: House Bill 166 - 133rd General Assembly Upon application by the holder of a license to practice as an oriental medicine practitioner or license to practice as an acupuncturist, the state medical board shall issue a duplicate license to replace one that is missing or damaged, to reflect a name change, or for any other reasonable cause. The fee for a duplicate license is thirty-five dollars.
