Effective: February 1, 2002 Latest Legislation: House Bill 279 - 124th General Assembly The deed or instrument constituting a person an attorney in fact shall contain:(A) The full name of the party appointing, and of the person appointed, and the residence and post-office address of each;(B) The number of the outstanding certificate of title, and the volume and folium of the register;(C) A description of the land that shall be the same as given in the certificate of title;(D) A brief statement of the powers conferred upon the attorney in fact.That deed or instrument shall be signed by the person making it and acknowledged as provided in section 5301.01 of the Revised Code.
