(a) In order to promote and maintain uniformity in the system of vital statistics, the forms of certificates, reports and other returns required by this article, or by regulations adopted hereunder, shall include as a minimum the items recommended by the federal agency responsible for national vital statistics, subject to approval of and modification by the State Commissioner of Health. (b) Each certificate, report and form required to be filed under this article shall have entered upon its face the date of registration duly attested.Laws 1963, c. 325, art. 3, § 310.
