(1) The Oregon Health Authority shall prepare forms entitled:\n(a) 'Declaration of Domestic Partnership' meeting the requirements of ORS 106.325; and\n(b) 'Certificate of Registered Domestic Partnership.'\n(2) The authority shall distribute the forms to each county clerk.
