Application for aid under ORS 412.001 to 412.069 shall be made to the Department of Human Services by the relative with whom the child lives. The application shall be in the manner and upon the form prescribed by the department. [Formerly 418.110; 2015 c.70 §9; 2015 c.765 §17]
