(1) The State Registrar of the Center for Health Statistics shall establish a Commemorative Certificate of Stillbirth. The certificate shall be signed by the state registrar.\n(2) The state registrar shall issue a Commemorative Certificate of Stillbirth for a stillbirth occurring on or after January 1, 1999, upon:\n(a) Request of a biological parent of the stillborn fetus; and\n(b) Payment of the fee adopted by the state registrar by rule.\n(3) The state registrar shall adopt by rule:\n(a) A form for the certificate;\n(b) The type of information that may be included on the form; and\n(c) The fee required for issuance of the certificate.\n(4) A certificate issued under this section is for commemorative purposes only and has no legal effect.
