(1) A licensee conducting a race meet, before selling a mutuel wagering ticket or receipt to any person about whom there is any reasonable doubt of the person having reached the age of 18 years, shall require such person to make a written statement of age and furnish evidence of the true age and identity of the person. The written statement of age shall be on a form furnished by the Oregon Racing Commission, substantially as follows:\n______________________________________________________________________________\nDate _____\nI am 18 years of age or over.\nSignature\nEvidence in support of age and identity:\nDriver’s License No._____ (_____)\nState\nMilitary Record No._____\nLiquor Permit No._____\nOther _____\n(Fill in license or card number of any one or\nmore of above)\n______________________________________________________________________________\n(2) A licensee who, in good faith and with reasonable cause to believe in its truth, accepts a written statement of age, as provided in subsection (1) of this section, may rely on the truth of the statement as conclusive evidence of the age of the person by whom it is signed.\n(3) No person shall make a statement of age, as provided subsection (1) of this section, may rely on the truth of the statement as conclusive evidence of the age of the person by whom it is signed.\n(3) No person shall make a statement of age, as provided in subsections (1) and (2) of this section, that is false in whole or in part, or produce any evidence that would falsely indicate his or her age.
