An application for a license renewal shall have the following characteristics: (1) The application shall be in writing, under oath and in the form prescribed by the department. (2) The application shall be filed at least 15 days prior to October 1. (3) The application shall include an update of the information under section 6212(b) and (c)(1) (relating to initial license application). (4) The application shall be accompanied by the following: (i) A new bond under the same provisions as set forth in section 6213 (relating to bond), which shall be filed annually at least 15 days prior to October 1. (ii) A license fee under the same provisions as set forth in section 6214 (relating to license fees), which shall be paid annually on or before October 1 for each license and place of business.
