When an attorney has appeared before a court in representation ofa client he shall, upon requesting withdrawal from said professional representation, include in his petition the last residential and postal address, including telephone number if any, of the person he represents and his own, set forth that he has notified the withdrawal to his client, and comply with Canon 20 of the Canons of Professional Ethics, App. IX of this title. The Clerk shall serve notice of the court’s order to the addresses furnished by the attorney. History —May 23, 1975, eff. Sept. 1, 1975.
