It shall be the duty of the Secretary of Justice to prepare the necessary blanks concerning the date specified under §§ 663 and 664 of this title, to be forwarded to judicial, administrative and police officials, both Commonwealth and municipal, who by reason of their offices are qualified to furnish the data therein required.
