(a) All applications for registration of a vehicle or hauling unit into the register, as well as all applications for issue of certificate of ownership, shall be submitted on the form provided by the Secretary for such purposes. All the information needed for duly registering or issuing the certificate of ownership shall be recorded on said form.\n(b) All owners of registered units shall be under the obligation to give written notice to the Secretary of any change of address within thirty (30) days following the date of said change by using the form provided by the Secretary for such purposes.
