(a) All the information in the file at the time it is requested, with the exception of information on the credit rating or any rating of risk, or predictions related to the consumer.\n(b) The source of the information.\n(c) The identity of each person who procures a report about the consumer. The name of the person or the name under which he/she does business and, at the consumer’s request, the address and phone number must be included.\n(d) Dates, payments and quantities of any check on which any information adverse to the consumer is based.\n(e) A record of all the applications sent during one preceding year about the consumer’s identity related to a credit transaction or insurance transaction that has not been initiated by the consumer.\nHistory —Sept. 2, 2000, No. 364, § 9, eff. 180 days after Sept. 2, 2000.
