Every association organized in accordance with this chapter shall file in the office of the Commissioner of Insurance, not later than March 31 of each year, a report duly signed and sworn to before a notary by the president or vice president and the secretary or manager, showing the financial condition of the association as of December 31 of the preceding year, and it shall be rendered in such form and shall contain such information as the Commissioner may prescribe.
