Once the contribution is received, the funeral home shall hand the contributor a duly-identified receipt in the amount of the contribution with the name of the funeral home and signed by an authorized signature. The funeral home shall also furnish a card identified as above, which the contributor shall give to the applicant (contracting party) of the funeral services in order to certify his or her contribution toward the costs of the funeral.\nHistory —Jan. 10, 1998, No. 23, § 4.
