Prior to the issuance of the certificate, the physician shall be convinced that the person requesting said certificate in writing, is the same person who is to contract marriage. If said person is unknown to the physician, he may be identified by a person known to said physician, who shall guarantee in writing, over his signature, that the person requesting an examination is the same person who is to contract marriage.
