Every subpoena shall be issued by the clerk under his hand and with seal of the court, specifying the part of the court, the title and the number of the action, and shall compel each person to whom it is addressed to appear and give testimony on the date, time and place specified therein.In the case of a subpoena for the production of documentary evidence, the clerk shall issue a subpoena like the one described above, but with a blank space.
