Every exclusive representative shall submit the following to the consideration of the Commission:(a) Within a thirty (30) day term from the date on which it is certified, a report containing the following information:(1) Name and address of the organization;(2) name and address of its officials and of the person authorized to receive notices and summons;(3) the amount required from its members for initiation fees, periodic fees, or service charges to unaffiliated employees, and(4) copies of its constitution, by-laws and collective bargaining agreements granted.(b) Each year, it shall remit a copy of its financial reports duly audited and certified by a certified public accountant, by an accountant or by the Labor Union Services Bureau of the Department of Labor and Human Resource[s]. These reports shall be submitted within a term of one hundred and twenty (120) days as of the closing date of their annual operations. Said reports shall be fled with the Commission remitted to the members of the labor organization.(c) After each election and within a term of fifteen (15) days counting from the same, it shall remit a report to the Commission that contains the names and addresses of
