(1) Complaints originated by the agency. — Each agency may file complaints before its administrative forum for infractions of the laws or regulations they administer.\nThe complaint shall contain:\n(a) The name and postal address of the complainant.\n(b) The facts about the infraction.\n(c) The legal or regulatory provisions for which the infraction is charged.\nIt may also contain a proposal for a fine or sanction which the accused may abide by and report his compliance or payment, as the case may be.\n(2) Complaints filed by a person outside of the agency. — The proponent of an action before the agency shall include the following information upon filing his complaint, application or petition:\n(a) Name and postal address of all parties.\n(b) Facts regarding the claim or infraction.\n(c) Reference to the applicable legal provisions, if known.\n(d) Remedy requested.\n(e) Signature of the proponent of the procedure.\nHistory —Aug. 12, 1988, No. 170, § 3.4, eff. 6 months after Aug. 12, 1988.
