Any of the following circumstances shall constitute grounds for denying or revoking a license or permit issued under this chapter:(a) Prior violations by the applicant or his/her representatives or agents, of any resolution, decision or order issued by the Secretary, or any provision of this chapter or the regulations promulgated thereunder.(b) Having furnished false information on the application.(c) Refusing to furnish pertinent information as required by the Department under this chapter or its regulations.
