The application to obtain a license to operate an agency shall be addressed to the Superintendent in writing, on the printed form supplied by him, signed and sworn to by the applicant or applicants, and shall be accompanied by sufficient demonstrative proof that all the requirements in § 285h of this title are met, and that the payment bond has been given in behalf of the Secretary of Labor and Human Resources as required in § 285f of this title.
