Application for obtaining the permit to carry out any of the activities set forth in § 563 of this title shall be filed with the Superintendent on the forms prescribed by him. Said application shall be sworn before any officer authorized to administer oaths, and shall contain the following information:(1) Name, address and age of applicant.(2) Name, address and age of the agent authorized by the applicant to receive, use, store, possess or transport the explosives or substances that may be used to manufacture explosives.(3) The place where and the purpose for which the explosives or substances that may be used to manufacture explosives shall be used, stored or transported.(4) The name and quantity of explosives for which permit is requested.(5) Signature of applicant.(6) Declaration of applicant and of each one of his agents where each of them shall state if he has been convicted of a felony within or outside Puerto Rico during the last 15 years.(7) Any other pertinent information that the Superintendent may prescribe by regulation.
